• Certification Details
• BLS, ACLS, and NIH Certification must be AHA and current.
• License/Certificate Verification required.

    • Job Requirements
    • Vaccines or Positive IGG Titer.
    • Fit Test: Required for all disciplines, Annual.
    • Hepatitis B documentation of vaccination series completion or documentation of declination to vaccine.
    • Influenza Vaccine: Required during flu season.
    • 2 Step PPD or IGRA (Quantiferon/Tspot): 1st within 1 year, 2nd within 30 days of start and/or Negative IGRA within 90 days of offered start date.
    • Varicella: Proof of immunity required; vaccines or titers if no proof.
    • Tdap: Valid 10 years from the administered date.
    • Ascension Skills Checklist: Clinical only; Required annually in specialty.
    • 12 Panel Drug Screen: Updated annually or upon break in service > 90 days.
    • Education Verification: Needs to be validated at the source level.
    • Nursys: License verification required for nurses.
      • Unit Specific Information
      • Ascension Unit Specific Checklist: To be uploaded by Ascension unit leader within a week.
      • Ascension Onboarding Requirements Education Attestation: Required pre-hire 6 hours.
        • Additional Details
        • MMR, Fit Test, Hepatitis B, Influenza Vaccine, PPD/IGRA, Varicella, and Tdap vaccinations or titers are required.
        • Ascension Criminal Background Check Results must be completed within 30 days of assignment start and updated every year or if break in service > 90 days.
